Patient Assistance Form

Our Patient Assistance Program helps with prescription medications, medical supplies, transportation to/from chemo and radiation treatment (gas cards), nutritional supplements, and more. To apply for assistance, download and fill out this form and drop it off at our offices.

Our Fundraisers & Events

Our fundraisers and events help The Louisiana Cancer Foundation provide practical support for cancer patients, hold free public cancer screenings, and fund cancer research and education in northeast Louisiana. Our yearly events include Cancer Foundation League Masquerade Ball, DBK Dance and Performing Arts Fashion Fusion, M.A.D. Dash 5K and Cancer Survivor’s Breakfast.
